RAIB has made 20 recommendations for the improvement of railway safety following the derailment at Carmont in August 2020. These cover: How the railway manages extreme weather events, management of civil engineering construction activities, management assurance of railway control functions, train design, and applying learning from previous events.
Commenting on the reports findings Andrew said;
"The conclusions of the RAIB report are as serious as it gets for the rail industry. It confirms the staff on the Carmont train acted impeccably and with every diligence.
"And it identifies a shocking chain of faults and failures which, if not visibly addressed, will undoubtedly impact on public confidence in our rail network.
"Going to the scene that day, and later witnessing the tributes to Christopher, Brett and Donald, the anguish of their friends and family, will stay with me forever.
"I commend the bravery of those who took part in one of the biggest multi-agency responses seen in Scotland.
"It shouldn’t have taken an accident of such grave scale to highlight what went wrong.
"Lessons must not only be learned, but acted upon swiftly.
